Checklist (for both CFL & Mediation matters)

The following document is available in both Microsoft Word format and Adobe PDF.

To download the Microsoft Word files:

1. Click on the appropriate download icon below.
2. When the document opens click on the File menu and choose Save AS.
3. Choose a location on your computer to save the file and then click the SAVE button.

To download the Adobe PDF files:

1. Click on the appropriate download icon below.
2. When the document opens click either on the "Save a Copy" button on the toolbar or on the File menu and choose Save.
3. Choose a location on your computer to save the file and then click the SAVE button.
